Home Care Package
Home Care Agreement
Upon choosing an aged care service provider that can meet your needs, you will be required to enter into a Home Care Agreement.
The Home Care Agreement is a legal document between you and the aged care service provider, it normally outlines the following:
- Care plan – services you will receive under your Home Care Package after consideration of your personal circumstances.
- Service delivery and cost – who will be delivering the care and services, as well as the costs involved in doing so.
- Budget – an overview of the funds that have been allocated to your Home Care Package, and how they will be spent.
- Rights and responsibilities – as governed under the Charter of Care Recipients’ Rights and Responsibilities for Home Care.
